Service Description

Need a work or school form filled out by the physician who certified you for medical cannabis? This requires a separate phone appointment for the doctor to ask you more specific questions pertaining to the form. Currently, we only fill out forms and do not write blanket medical clearance letters. When making your appointment, make sure to select the same physician who did your certification. If you are unsure of who this was, you can either check your booking confirmation email at the bottom or login to the state portal to view your certification. All forms should be mailed to info@justmydoc.com in advance and will be completed during the time of the appointment. They will be completed and sent to you and any designated recipients by the end of the session.
